Scheduling Policies

Pre-appointment details

Intake documents will be provided electronically to patients and clients for completion prior to appointments. Paperwork includes Intake Form, Media Release, and Policies and Procedures.

Cancellation policies

Appointments will be scheduled on a first-come-first-served basis. Patients and clients are advised to cancel at least 24 hours prior to appointment time. Additional details may be found in the Policies and Procedures document.

Payment policies

For private pay services, payment is due at the time of service. Payment is acceptable as electronic payments transfer, credit card and personal checks payable to Brave Babies LLC.
